29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: NB Siding Construction LLC at 2152 Fountain Cir. Erie, CO 80516: On or before 11/1/18 the creating and exposing employer did not protect employees from injury in that training was not provided to employees by a person qualified in the subject matter to recognize hazards associated with the use of pump jack scaffolds. An employee fell from a pump jack scaffold due to the decking giving way, as one side of the pump jack scaffold was lowered too far in relation to the other side. This condition exposed employees to an unsafe scaffold and a fall hazard of 15 feet.
