29 CFR 1926.1053(b)(6):Unsecured ladder(s) were used on surfaces that were not stable and level: (a) Main ST. and US-51, Madison, MS. - On or about July 22, 2025, the employer had employees use the fiberglass extension ladder to gain access and egress the trench the was over 6 feet deep on unstable and level surface without securing it to prevent displacement exposing employees to fall hazards.
