29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about October 24, 2023, employees were exposed to a fall of 22 feet while performing roofing activities on a residential home located at 3817 Goldenwood, Uniontown, Ohio 44685. The employees were using a portable ladder to access the roof, and the employer did not ensure that it extended three feet above the landing area.
