29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. a) Shear department - On or about 1/30/2020, and at times prior, the employer exposed employees to struck-by and amputation hazards in that no procedure was utilized for the control of hazardous energies when employees were repairing the machines such as, but not limited to, Amada shear machine (Serial # 4045151 and Model # M-4045).
