29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems: a) Sewer Solutions located at 1374 Yates Street, Denver, CO 80204: On and preceding September, 20th 2018, the employer did not ensure that daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins. Workers were digging and working in a trench which was up to approximately 12 feet deep to tie in to a main sewer line for a new construction multi-family dwelling. The following condition exposed employees to struck-by and caught-in/between hazards.
