29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): (a) Quasar Energy Group, LLC on or about May 09, 2015, at the address of 2350 Abbe Rd. Sheffield Village, OH: The employer failed to assure that a hazard assessment identifying and selecting appropriate personal protective equipment was conducted prior to the performance of maintenance work atop of the unguarded digester tank. An employee performing maintenance activities from the elevated surface prior to a task specific employer assessment was exposed to falls of approximately 30 ft. to the ground below.
