29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons could accidentally walk were not guarded by a floor hole cover of standard strength and construction: (a) Primary Mud Tank: Employees working in the mud tank area were exposed to trips and falls from an unguarded floor hole measuring approximately 5 3/4 inches by 3 3/4 inches, as observed on or about 12/02/2014.
