29 CFR 1928.57(b)(2)(ii): All revolving shafts, including projections such as bolts, keys, ore set screws, shall be guarded, except smooth shaft ends protruding less that on-half the outside diameter of the shaft and its locking means. On or about September 30, 2018 an employee was operating a twelve row self propelled ear corn picker without guarding of the shaft from the main belt drive to the gear drive of the husking rolls. This condition presented a caught-in hazard that could result in an entanglement of employee clothing resulting in a serious injury. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
