29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a)Line #1, Press Machine #3; On or about October 7, 2020 and at times prior to the employer required that employees conduct maintenance activities such as mold change out without ensuring that procedures to control the energy sources were utilized by employees exposing employees to crush-by hazards.
