29 CFR 1910.67(c)(2)(xi): The insulated portion of an aerial lift was altered in a manner that might reduce its insulating value. The employer does not ensure that the insulated portion of an aerial lift is not altered in a manner that might reduce its insulating value. This violation occurred on or about September 26, 2014, on the north side of a building where an employee was exposed to electrical and fall hazards when trimming a palm tree from a vehicle mounted Hi-Ranger (SN: 77910228) aerial lift that had the insulated portion of the boom altered and covered with plywood that was screwed into the boom. Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the insulated portion of an aerial lift is not altered in a manner that might reduce its insulating value.
