Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees a. Located in the weld shop, employees performed welding and cutting on crude oil storage tanks without utilizing a combustible gas detector to monitor for flammable, combustible, or other hazardous vapors before and during the hot work to ensure vapors were below their lower explosive level. The employer utilized a system that included the purging of tank with nitrogen, but did not utilize an oxygen analyzer to ensure that the oxygen had been displaced to a level of essentially zero. The employees were exposed to an explosion hazard by not ensuring that the atmosphere in and around the tanks was safe for hot work to commence.
