29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. (a) Employees repairing a roof at 3615 Gardner Ave, Kansas City, Missouri, were not provided with personal fall protection, covers over skylights, or guardrail systems to protect them from falling through skylights that were 21 feet 7 inches above lower levels. An employee was fatally injured when he backed onto and fell through an unguarded skylight. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
