Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: (a) On or about January 19, 2013, for employees engaged in well servicing operations including but not limited to fracturing operations where the employer failed to secure, restrain and/or anchor the open-end of the bleed-off valve discharge line (pressure line) from movement, resulting in struck-by hazards from the uncontrolled movement at the Kodiak oil site, KOALA 154-97-15-34-27-1H3, Watford City, ND. Abatement Note: Among other methods, feasible and acceptable abatement methods to correct the hazard would be for the employer to secure and/or restrain the open-end, bleed-off valve of discharge lines (pressure lines) from movement in accordance with American Petroleum Institute ("API") Recommended Practice 54, Third Edition, "Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations", section 12.4, DISCHARGE LINE (TEMPORARY TREATING OR CEMENTING LINES), subsection 12.4.3, which states: "When using an open-ended flow line to flow or bleed-off a well, the line should be secured at the wellhead, at the end of the flow line, and at intermediate intervals along the line to secure each joint. The flow line should be anchored prior to opening the wellhead control valve."
