A.R.S. 23-403.A: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees had not been trained in fall protection and ladder safety and were required to climb portable ladders to access walking-working surfaces that were nine feet above the ground level. (a) 2770 E. 13th St, Yuma, Az: Employees working on walking-working surfaces that were nine feet above the ground level were not trained in fall protection. (b) 2770 E 13th St, Yuma, Az: Bulk water delivery employees were required to use sixteen foot long portable extension ladders and were not trained in ladder safety.
