Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: TELECOMMUNICATION TOWER CONSTRUCTION SITE, 2363 S. Denmark Rd., Jefferson, Oh.: On or before April 14, 2000, the employees, who were working on the job site, were exposed to the hazard of being struck-by materials, equipment and falls due to failing hoist and gin pole components. Among other methods, feasible and acceptable methods to control the hazard are: (1) Conspicuously post rated load capacities, recommended operating speeds, and special hazards warnings on the hoisting equipment. (2) Post personnel load capacities for the current configuration of the gin pole and related equipment. (3) Assure operators are trained in the requirements of OSHA directive 2-1.29 prior to utilizing base mounted drum hoist to lift employees to work stations. (4) Inspect thoroughly the gin pole and related components and maintain at the job site all inspection records. (5) When a anti-two block cannot be used, maintain on site a specific program to ensure that two blocking does not occur and that effective communication between the hoist operator and the personnel being hoisted is maintained. This program could include a cable marking system, an employee on the tower in a position to observe the top block.
