OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a. On or about August 17, 2022, an employee was exposed to a struck-by hazard due to the unintentional actuation of a 5000-pound forging hammer (#409) while attempting to remove a part being forged that was stuck in a die during the last stage of the forging process. Machine #409?s foot treadle required 50-55lbs of continuous, downward pressure to move the treadle from its raised position 6 inches down to cause one die to contact the other die.
