29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a. At a worksite located at 4900 Mobile Highway in Pensacola, Florida, on or about June 26, 2020, the employer exposed employees to crushing hazards, in that the employer did not ensure that all powered industrial truck (forklift) operators were provided with operator training while performing powered industrial truck operations.
