29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Woodsage, LLC. located at 7400 Airport Highway in Holland, Ohio; On or about January 27, 2020 the employer did not ensure that employees were protected from amputation hazards while testing the roller assembly machine #2 in CAP mode. On January 27, 2020 an employee sustained multiple fingertip amputations on their left hand while attempting to remove the McMaster-Carr roller when the ejection pin was automatically triggered.
