29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On September 25, 2019, at approximately 5:30 pm, an employee was riding a company owned shuttle bus from the hospital building to their personal vehicle located in the company parking lot. The employee slipped on steps while exiting the shuttle bus and fell to the ground suffering an injury requiring hospital admission. The employee's supervisor was informed of the injury and hospital admission at this time but the admission was not reported to OSHA until September 30, 2019.
