Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: On or about August 27, 2018, at the dust collector, the employer failed to ensure potential energy was rendered safe. The employer did not have in place energy control procedures to stop the paddle wheel from turning from the weight of the sawdust in the hopper. Employees exposed to an amputation hazard from being struck by the unexpected turning of the paddle wheel blade from the gravitation force of the sawdust in the hopper.
