29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a. On or about 11:15 am on 04/17/2018 at 2220 Cimmaron Terrace, Palm Harbor, Florida, an employee suffered a work related finger amputation. The employer was aware of the amputation on the same day of the incident. The employer notified OSHA on 04/20/2018 at 5:40 am of the amputation.
