Text For Citation: 01 Item/Group: 001 Hazard: HEAT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about August 15, 1998, employees were exposed to the hazard from escaping steam and liquids and injuries and lacerations from flying debris and broken glass when the jacketed steam heated kettle exploded when the discharge became plugged or restricted. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS...." To use pressure reduction valves prior to the kettle inlet and a pressure relief device (safety valve) on the reducer bypass and/or on the steam heated kettle. Additional safety is required on pipe valve and fitting sizing in that the inlet (high pressure) discharge openings must be significantly less than the larger openings at discharge as stipulated in ASME, boiler and pressure vessel code or Wisconsin boiler and plumbing code.ing
