Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and/or falls from an overturned aerial lift: Prince Crossing and Route 38. Aerial lift was used on a slope exceeding 5 degrees without extra precautions taken or any instruments available to determine the slope. Feasible and acceptable abatement methods include but are not necessarily limited to: 1.Install an aerial lift level indicator available from the aerial lift manufacturer or other vendor. 2.Provide specific safety instructions to the employees to be incorporated into Ameritech's safety program when working on slopes over 5 degrees. These can include having the boom not extended to full reach or at a horizontal level, and having the boom centered over the cab. 3.Provide a determination of maximum slope allowable for the aerial lift that cannot be exceeded under any circumstances.
