29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about March 7, 2017, the employer did not ensure that the feed opening to the catapuller #8 was guarded in such a manner as to prevent the employees hands from reaching the nip point. The diameter of the rubber cone was large enough such that employees were exposed to in-running nip points and at risk of injury.
