29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a-h) Saw line section, pop-up saws 1-8: On or about August 11, 2015, and at times prior thereto, employees were exposed to hazards associated with unexpected startup while removing saw dust or replacing the cutting blade on the pop-up saws. (i) Saw line section, chunker machine: On or about August 11, 2015, and at times prior thereto, employees were exposed to the hazards associated with unexpected startup while cleaning out the jams on the chunker machine.
