29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Quasar Energy Group, LLC on or about May 09, 2015, at the address of 2350 Abbe Rd. Sheffield Village, OH: The employer failed to assure an employee servicing the deflated rubber membrane roof atop the 30 ft. tall digester tower tank was protected from a fall. The employee working from this unguarded elevated surface in this condition was exposed to an approximant 30 ft. fall to the ground level below.
