Investigation Summary
At 3:30 p.m. on May 29, 2013, an employee was in an excavation with another coworker waiting for a trackhoe to return, so that they could connect cables to steel panels for the removal process of a slide rail shield system. It started to rain, and the employee attempted to shield himself from the rain by squatting underneath or behind a steel panel that was not installed correctly per manufacturer's recommendations. The employee was struck by a steel panel of a slide rail shield system in an approximately 40 foot deep excavation thus killing the employee.
