Investigation Summary
At approximately 9:30 a.m. on February 13, 2014, Employee #1 was repairing the rear differential of a Volvo LB35B Pro wheel loader, Serial Number 1876049, and was reinstalling the rear axle and related hydraulic fittings when the loader moved suddenly and fell off of the jack stands that were supporting it. When it came off of the stands, it fell directly on top of him, causing blunt force trauma injuries to his head and neck. Employee #1 died on-site.
