Investigation Summary
On January 10, 2014, Employee #1 was cleaning a horizontal tilting food mixer. The employee was reaching into the unit with one hand and at one point with both hands with the mixer in the tilted configuration and with the cover open. The mixer had one identification plate on it marked "Tops" but no other identification such as model numbers or serial numbers could be located. The employee was wearing shoulder length gloves at the time and had been using a common abrasive pad. Employee #1's right index finger was caught by the mixer. The employee was taken to an area hospital, where he was treated for a laceration to the right index finger requiring amputation.
