Investigation Summary
At 8:45 a.m. on May 8, 2012, Employee #1 was applying adhesive to the edge of the roof, and he fell from the edge. The adhesive was being applied in preparation to install weatherproofing paper on top of the existing roof. Employee #1 was not wearing any type of fall protective equipment and there was no fall prevention equipment on the roof. Employee #1 was not wearing work boots at the time of the accident, but he was wearing sneakers.
