Investigation Summary
Employee #1 was sorting grape leaves out of the rear bin that loaded on a trailer. A coworker, operating a tractor to pull the trailer loaded with two one-ton bins, failed to ensure that all other coworkers in the surrounding area were clear of the tractor prior to movement. Employee #1 was subsequently run over by the trailer when the tractor operator attempted to move the trailer to another location.
