Employee Falls from Roof, Fractures Head and Shoulder

On September 24, 2008, Employee #1 and five coworkers were installing a new roof on a two-story single family dwelling. At the time of the accident, Employee #1 was standing on the corner of the first story, removing old roofing material when he stepped into a pile of roofing debris, slipped and fell 10 ft. Employee #1 suffered head and shoulder fracture injuries and was hospitalized.
