Investigation Summary
At approximately 11:55 a.m. on January 24, 2006, Employee #1 and a forklift operator were unloading two 7-ft, and three 10-ft high roof trusses from a 6,000 lb. capacity rough terrain forklift. When the employees removed the second 7-ft truss, the three 10-ft trusses fell, striking Employee #1 in the shoulder. They knocked him to the ground, unconscious. The three larger trusses were estimated to weigh 800 lbs. and landed across Employee #1's chest, pinning him to the ground. Employee #1 was hospitalized.
