Employee killed when wall collapses

On Septemner 12, 2003, Employee #1 was applying waterproofing on the inside areas of a 9 ft tall block wall when it collapsed. He was pinned to ground and died as a result of his injuries. The wall had been previously backfilled, but had not been braced or supported in any other way to prevent a collapse.
