Employee Received Survery For Repetitive Motion Injuries

On May 17, 2001, Employee #1, a full-time employee working as an image processor (now known as a supply chain management analyst) sustained an initial injury on her right hand/forearm due to a Repetitive Motion Injury (RMI) from computer use. The injury then moved up to her right shoulder and neck area, and an additional injury report was filed on April 29, 2002. A surgical procedure, a cervical disk replacement and disk fusion in her neck, occurred on January 30, 2003 at Citrus Valley Medical Center in West Covina, CA.
