Investigation Summary
Some employees were in the progress of assembling a dock system called Ez-dock. To install the dock, one employee holds a bolt three feet from the edge of the section added and another employee attaches a nut at the top. The employee holding the bolt was wearing a snorkel mask and dived under the dock area by holding his breath momentarily in the water then he would come out to the side. For some unknown reasons, the employee under the dock failed to come out of the water to the side of the dock. He drowned.
