Investigation Summary
Employee #1's employer and a coworker worked on the top of the north tank. Employee #1 worked from a bracket scaffold, which was about 13 feet above the ground and bolted to the north tank. A contractor was putting the top on one of two 35,000-gallon-steel-bolted water tanks. Each tank was 16 feet high and 18 feet in diameter. Employee #1 went down to the ground to get more bolts. When he returned to the scaffold, he did not use any fall protection and fell off the scaffold. He landed on the concrete on the right back side of his head and died from craniocerebral injuries.
