Investigation Summary
At approximately 4:55 a.m. on November 30, 2001, Employee #1 was operating an ACOP 25SL Electric Side Loading Order Selector in a narrow aisle. He had loaded a denim roll on to the conveyor system and was in the operator's platform at a height of approximately 25 ft when he began traveling backwards toward the entrance of the aisle. He traveled approximately 34 ft with the load raised when he struck another denim roll protruding out of the aisle. The conveyor system, mast and operator's platform snapped from the base frame on the rear of the vehicle causing Employee #1 to fall 25 ft to a concrete floor. Employee #1 died.
