Investigation Summary
On March 1, 2008, an employee was wiping a stamping die that was on the forks of a motorized transporter. The employee was using a rag to remove dust and chips. As he was wiping the die, it suddenly shifted to the right and fell off the forks of the transporter. The employee jumped back to prevent the die from falling on his feet when he realized that his right fifth finger tip had been amputated. The employee was a set-up person for the punch presses. The employee was taken to Whittier Presbyterian Hospital where he was treated for his injuries and released.
