Investigation Summary
At approximately 7:00 a.m. on January 29, 2004, Employee #1, a 58-year-old carpenter, was working on a residential home construction site. He was located on the roof of a two-story building wearing a fall protection harness and lanyard and was walking on OSB roof sheathing to nail a harness ring to the peak of the roof to tie-off. He slipped, sliding off the roof and fell 21 feet to the ground. Emergency services were contacted and Employee #1 was hospitalized at the Inland Valley Medical Center in Wildomar, California. He was treated for multiple injuries, which included: blunt chest trauma with several rib fractures, pelvic fractures, and internal injuries. He was transferred twenty one days later to Casa Colima Centers for Rehabilitation in Pomona, California.
