Investigation Summary
At 12:30 p.m. on December 6, 2002, a serious injury occurred at a manufacturing plant for packaging material. The employer had 60 employees and was non-unionized. Employee #1 was a temporary employee of the employer. Employee #1 was working on a packaging machine, a laminator made by Alpha Marathon Castline, Serial Number 243A-05-02, CAT-AMT80BFL. The machine was running in normal operation mode. Employee #1 noticed some material had gotten stuck and it would not let the machine run. As Employee #1 tried to remove the jammed material, his left hand got caught between the hot rollers, about 450 degrees Fahrenheit, and as he tried to pull his left arm with his right hand, his right hand and arm got pulled in between the rollers. The accident resulted in third-degree burns and multiple fractures on both his hands and arms.
