Investigation Summary
On June 7, 2002, Employee #1 of Speedway Metal Finishing was buffing a circular part of an automobile wheel using a floor-mounted M-C 10-HP polishing lathe. To do this, he was holding the part ringed around the rotating polishing wheel. The circular part with 13.75 in. inner diameter got stuck the 12-in. buffing wheel, which caused Employee #1 lose his balance, and the rotating part hit him in the head multiple times. He suffered serious skull fracture. He was taken to Western Medical Center in Santa Ana for treatment. As a result of his injuries, he died on July 20, 2002.
