Investigation Summary
On January 13, 2001, an employee was filling bags of flour when he noticed that the flour was not coming out of the machine. He went outside to the baghouse to see if the flour was being obstructed in any way. The employee placed his right hand into the dropper, which consists of several blades. He amputated three of his fingers. The employee was hospitalized for three weeks for his injuries. One finger was successfully reattached. Several days prior to the accident, maintenance had changed this area from a closed system product flow to an open system. The open system generates reject flour that drops out of the system into barrels to reduce contamination. When this changeover was completed, maintenance failed to replace the dropper guard.
