Investigation Summary
At approximately 9:00 a.m. on January 23, 2003, Employee #1 was working with a coworker installing cabinets in a new residential home construction project. Employee #1 was cutting (ripping) a piece of wood (particle board) that measured approximately 3 in. by 2.5 in. to be used as a filler for the cabinet. Employee #1 was using a saw that he owned (De Walt Portable Table Saw DW744 Blade 10" 250MM 3,650 RPM Type1 120 volt AC Serial Number 53511.) Employee #1 was cutting the wood and heard a noise and looked in the direction of the noise. Employee #1 looked away but he still continued to cut the piece of wood. Employee #1's fingers came in contact with the blade of the table saw causing an amputation. Employee #1 was hospitalized.
