Investigation Summary
On December 8, 2006, Employee #1 was working for a landscaping and tree service. A wood splitter used by the firm had a flat tire and had gone to the repair shop to have the flat fixed. As Employee #1 was unloading the wood splitter, it slipped. A plate on the wood splitter landed on his the tip of the index finger on his left hand, amputating it. He was hospitalized.
