Investigation Summary

At 8:13 a.m. on October 5, 2020, an employee was preparing to string cast extrusion line 25. The employee went through the light curtains and ducked under the winder spindle. The employee proceeded further into the machine and stepped onto the inbound roll changer safety mat that was in front of the spindle. The employee found the spindle rope tangled and needed to untangle it to continue threading the line. When the employee stepped on the safety mat, the carriage moved forward causing the rider roller to fall forward and strike the employee on the head. The employee then fell to the floor striking his head on the ground. The employee was transported to the hospital and died from the head trauma three days later.

Investigation Summary

At 11:39 a.m. on July 25, 2017, an employee was performing some maintenance on a printing press armiture and opened an access panel. The employee stuck his head inside the access panel and a coworker started the equipment, not realizing that the employee was in the machine. The employee's skull was crushed and the employee killed.
