Investigation Summary
At 9:00 a.m. on April 27, 2020, an employee was adjusting a 2 in. by 4 in. piece of wood that was being used for a spacer on a fork truck to push material onto ta truck bed. With his hand on the piece of wood, a coworker (the branch manager) contacted the forklift forks and the employee's left index finger was pinched. The employee's left index finger was partially amputated. The employee was treated without hospitalization.
