Employee Pinky Finger Is Amputated In Machine

At 6:55 a.m. on August 19, 2019, a coworker was operating a case packer. The cow orker heard a noise, and the case packer was not operating correctly. The cowork er hit the e-stop on the machine, and called Employee #1 to troubleshoot the mac hine. Employee #1 opened the machine to try to unjam a box. Employee #1 then clo sed the machine, and told the coworker to reset the machine and activate the air to the machine. Employee #1 stuck his hand under the door to activate a limit s witch, forcing the machine to cycle. Once Employee #1 activated the limit switch , the machine cycled and a piston plate went up and smashed Employee #1's pinky finger. Employee #1 was hospitalized to treat a right pinky finger amputation.
