Investigation Summary
At 2:30 a.m. on January 7, 2019, an employee was standing on a work platform att empting to unjam boxes on a box conveyor. The employee pushed the jammed boxes w ith a metal pole provided for that purpose when he slipped and lost his balance. He fell down a stairway opening hitting the conveyor frame and/or the floor. Th e employee suffered a skull fracture and hemorrhaging and was hospitalized.
