Investigation Summary
At 8:50 p.m. on October 2, 2018, Employee #1 was cleaning the in-running dough rollers of a pasta dough machine with a rag. He did not lift the guard, which stops the rollers. The rag, followed by his two fingers of his left hand were pulled into the in-running rollers and crushed. The two fingers had to be surgically amputated and the employee was hospitalized for treatment.

Investigation Summary
At 3:20 p.m. on February 5, 2018, an employee was on top of a 9 foot wooden framed wall in a building that was under construction and was nailing braces on the top portion of the wall to the plumb wall. The employee turned to nail the next brace, lost his balance, and fell from the wall to the floor, landing on top of his hammer. The employee was hospitalized with lower back pains and a sprained left hand.

At 1:30 p.m. on November 16, Employee #1 was sweeping the ground. A coworker, a forklift operator, was transporting a 3-yard trash bin on the forks, and was moving to exit the building.
